With the form, I think we need to offer Gender as Male, Female and Non-binary.
I think the word OTHER can be viewed as offensive to those.

I have taken the liberty of removing this field altogether for all forms on the basis that it is not information the RYALT needs to provide the service. Let me know if this is incorrect. |

I think for a community organisation that is contacting us about starting a literacy hub-they need a different type of registration form to a student form.
We should remove the DOB/Gender from this form.
Add business name under Full Name as an option
Keep the best time to call toption but remove what phone company are you with

Have updated the form as requested and added a company/organisation name and position field. Also DoB has been removed for this service. |

Again, I feel we need a different type of registration form for Workplace literacy training that doesn’t include DOB/Gender etc.
It really should be more of a Contact Us form so that we can get in touch with them.

Have updated the form so that gender has been removed on all forms – I assume this is not required for the purposes for which the form is being submitted. I have also hidden DOB for workplace staff training and added name of company and position. Let me know if this is now ok. |
